Nursing Facility Transition Program  

A Medicaid based program for persons seeking alternatives to nursing facility residency

Supports Coordination

The Nursing Facility Transition (NFT) staff assist individuals in Jackson, Hillsdale,Lenawee and Washtenaw counties who reside in a nursing facility and whose care is paid for by Medicaid, to return to community-based living.

The NFT staff works closely with the individual to determind their individual  needs to ensure a successful transition.  This *may include: 

  • Move out of Nursing Facility 
  • Identify and locate affordable and assessible housing options 
  • Identify necessary in-home services 
  • Durable medical equipment
  • Adaptive equipment 
  • Environmental and some home modifications
  • Grab bars and safety equipment
  • Transportation and bus line information
  • Bridge card re-application support 
  • One time grocery support 
  • Household items/ personal care/ furniture support 
  • Advocacy support 
  • Ramp infomation and support 
  • Community resource support 
  • Utility resources and support
  • 6 month follow-up after transition

An individual may terminate this process at anytime if he/she chooses to remain in the nursing facility.
