How to Apply

Loan Closet

Whether it's a temporary or permanent disAbility, equipment can be a major hurdle.  Our loan closet offers wheelchairs, walkers, commodes, canes, lifts, exercise bikes, grab bars and geriatric chairs, plus much more.  Please pick up equipment at the front door.  Equipment returns at the back door (ring buzzer).  Personal equipment repairs are available.

Loan Terms & Rates

Recommended Charges (non-refundable)

Rates (effective 3/04) for loan closet items are due at the time of checkout (cash, check and money orders accepted) and are as follows:

• High Tech Items: $25 per month (photo ID needed)
  Low Tech Items: $10 per month

• Motorized Equipment: $25 per week (photo ID needed)

• Manual Wheelchairs: $15 per week

• Lift Chairs: $50 per month

• Hoyer Lift: $25 (Powered $50 per month)

• Walker w/Seat, Geri Chair, Extended Bath Bench, Commode: $15 per month

• Commode Pail: $5 (one-time fee)

• Shower Seat (with or without back): $10 per month

• Walkers (with or without wheels), Accessible Strollers, Adult & Child's Crutches, Quad Canes, Bathtub Rails, Toilet Seat Risers, Toilet Rails, Bedside Tables, Bed Wedges, Child's Tumble Form Feeders, and small equipment for personal home use: $10 per month

Terms
1.  Equipment loans are for short term use.

2.  If equipment is needed longer than 30 days, the next month's fees are due and payable.  Consumer will be responsible for notifying disAbility CONNECTIONS, Inc. for extension request.

3.  Loan item(s) remain the property of disAbility CONNECTIONS, Inc.

4.  Consumer agrees that all item(s) received in good repair and clean condition.  Consumer will return them in the same condition or pay for any damage/loss.

5.  The individual using the equipment assumes full responsibility and agrees to indemnify (hold harmless) disAbility CONNECTIONS, Inc. and it's directors, employees and agents, from all liabilities arising in any way from the use of equipment.

Additional Terms

If delivery/or pickup is necessary, $25 per trip + mileage (service available for Jackson County only). No delivery service for tech items or toys. Because of limited quantities of many items, we can only hold item(s) up to 48 hours.  There is a loan waiting list available, but we do not know the date of potential loan request fulfillment.
